Oconee Regional Medical Center

Admission Information

Admission Information and Procedures

We are pleased that you and your physician have placed your trust in us to provide your medical care. Our team of health care professionals is committed to doing their best to make your visit as pleasant as possible.

The hospital’s admission office is located in the main lobby of Park Tower. If you need additional directions once you get to the hospital, please stop at the Information Desk in the Park Tower lobby. Call 478-454-3500 if you have any questions regarding your admission to Oconee Regional Medical Center.

Patients or their immediate family members are requested to provide the following information and documents, as it applies to you:

  • Insurance cards;
  • Social Security numbers for patient and spouse, and, if a pediatric admission, for parent and patient;
  • Retirement dates for patient and spouse (Medicare patients);
  • Birth dates for parents (for pediatric admissions);
  • If you are being admitted, Living Will and/or durable power of attorney (Medical Power of Attorney) if you have completed these forms.

Federal regulations require hospital personnel to inquire as to the presence of an Advance Directive. This includes a living will or durable power of attorney. Information on Advance Directives is provided to every patient upon admission, and may also be found by clicking here.

Please note: Due to the nature of these materials, we suggest that you take time to read and make decisions about them in the quiet of your home, before coming to the hospital. Georgia laws relating to the witnessing of these documents make it difficult to sign them after admission to the hospital.

If you have a pre-scheduled appointment for admission, it is important that you arrive on time, because your physician may have scheduled special tests for you (i.e., blood, x-rays, etc.) You will be asked to sign a consent form for admission to the hospital and for medical treatment.

An identification bracelet for your wrist will also be prepared. At this time you will be taken to the nursing unit to which you are assigned. A nurse will interview you by asking questions about your general condition, diet, allergies, personal habits, and other factors that will apply specifically to the nursing care you will receive.
